Initial Assessment & Summary  $575

Please allow 90mins face to face for initial assessments when engaging with the FOHT for the first time. Initial Assessment includes Teacher/parent/Carer report and informal assessment and Summary Report from treating Occupational Therapist. This does NOT include formal assessments.

Comprehensive FunctionAssessment  (Up to 20 Hours

Comprehensive assessment including functional assessment will be billed at the time taken to complete the assessment and report.

Session Fees (Private & NDIS)

OT: 45 mins      $193.99 plus travel

Includes 45 mins of face to face therapy time and 15 minutes for intervention planning. Including recommendations for activities and supports and also liaison with teachers/other relevant people in your life.

Travel Fees

Whilst we try to keep our travel fees as reasonable as possible, to ensure that families are getting the most value for their therapy, travel fees do apply.

Offsite work will an incur a travel fee. Our travel fees are $193.99 per hour. Please note this fee will be divided between clients if our therapists is visiting other clients at that location.

Prices vary for all individual assessment measures, and report summary is included within assessment price. Please don't hesitate to contact us for a price guide.

Documentation Fees 

$96.99 per 30 Mins

Includes requested letters, application forms, individualised progress report at the end of the block, etc.

Please contact Jana-Jade Loadsman at the Foundation of Hope Therapies if you require advocacy support and are experiencing financial difficulty. As pro-bono services will always be allocated with priority.

Whilst we do understand that families can forget to meet scheduled appointments, cancellation fees DO APPLY if you do not advise of Non Attendance with more than 48 hours notice prior to your session. This allows others to take up your allocated session time.

Cancellation Fees**

100% of session cost will be charged to be used to cover OT session preparation time.
